DISCLAIMER
At the time of booking your new patient consultation with Contempo Aesthetics, we will secure a non-refundable deposit that will be applied towards your first service. If you wish to schedule for same-day treatment, as a new patient, a larger deposit will be secured in order to hold sufficient time for the consultation and service. Services require a deposit comensurate with overall service cost. All deposits, purchases, and sales are final. No refunds will be honored. As with all medical treatments, there is no guarantee of outcome. While medical treatments are tested through clinical studies (http://clinicaltrials.gov) to determine their safety and efficacy, there are always going to be a percentage of people who do not see efficacy with any drug/medical device.
PAYMENT
Contempo Aesthetics does not accept health insurance or personal checks. Contempo Aesthetics accepts cash, Visa, MasterCard, Discover, and American Express as methods of payment. We also accept some HSA debit cards and Care Credit. Services must be paid in full prior to receiving treatment(s). Please be prepared to present Government Issued I.D. at the time of payment. I.D. and credit card must match names. No exceptions. ALL SALES ARE STRICTLY FINAL. NO REFUNDS. NO EXCEPTIONS.

CARE CREDIT
Contempo Aesthetics accepts cash and credit cards only for services, however we are pleased to offer CareCredit as a financing option for clients. CareCredit is a healthcare-specific credit card accepted by a variety of medical offices. Approved cardholders may take advantage of special deferred interest payments on services over $1000 for 6 or 12 months or elect for extended pay, interest-bearing options. Care Credit promotional rates may not be combined with any other offer including client referral discount or flash sale purchases.

CONSULTATIONS
NOTE: IF YOU HAVE NOT BEEN SEEN BY OUR PRACTICE WITHIN THE PAST 2 YEARS TO THE DAY, YOU WILL BE CONSIDERED A NEW PATIENT AND SCHEDULED ACCORDINGLY. Doctor Rachel is dedicated to helping clients achieve their aesthetic goals. All consultations require a non-refundable deposit that will be applied towards your first service(s). All consultations are scheduled for 30 minutes, with approximately 15 minutes of dedicated provider-patient face-to-face time. For same day service, the deposit is higher as time is allocated to service. Please note that services are only performed following receipt of signed patient consent forms and collection of clinical before and after photos.
NO GUARANTEES
With all medical devices and pharmaceuticals, there are no guarantees in medicine. While medical treatments are put through clinical studies (http://clinicaltrials.gov) to determine their safety and efficacy, there are always going to be a percentage of people who do not see efficacy, with any drug/medical device. As stated in the medical literature in which clinical studies are published, not everyone will respond the same to treatment, and the results depend on a variety of factors such as degree of laxity of the treated skin, healing response of the patient, and genetic predisposition.
SCHEDULING
Appointments, including consultations, must be booked in advance. Walk-ins may be turned away. We highly encourage bookings be made online. At the time of booking, please be prepared with your credit card.
CONFIRMATION CALLS, EMAILS, AND TEXTS
A confirmation call will be placed from our office approximately 48 hours prior to your appointment. Automated confirmation emails and texts are sent 48 hours prior to your scheduled appointment time as a reminder. However, it is your responsibility to maintain your appointment.
ARRIVAL
Check in with the front office at least 5 minutes prior to your scheduled appointment to prepare for your treatment. As we are a medical clinic, we require that all new clients fill out health history intake and treatment consent forms. Intake forms are emailed for electronic submission 24 to 48 hours prior to your appointment.
NON-PATIENT POLICY
Please note that non-patients or patients not receiving a consultation or service will not be permitted inside the treatment room to ensure that the environment be preserved for utmost privacy and safety. This rule also applies to children and babies in strollers. Be sure to make the proper accommodations prior to your appointment.
VALUABLES
We are not responsible for lost or stolen items, and encourage you to leave valuables at home.
LATE ARRIVALS
Our scheduling is designed to permit the correct amount of time to complete your service. A late arrival will deprive you of precious treatment time. In fairness to others, your treatment must end on time so the other clients ‘ sessions can begin on time. If you are not able to be on time, we will do our best to complete as much of your treatment as possible; however, with some treatments, it may be necessary to reschedule your appointment. If you are 10+ minutes late for your scheduled appointment, you will be considered a ”no-show” and charged for 100% of your service fee.
CANCELATIONS
Your scheduled appointment is reserved exclusively for you. Should you need to cancel or reschedule your appointment, please notify us at least 48 hours in advance to avoid losing your deposit(s). All services canceled or rescheduled the day of appointment will be charged a $75.00 fee for a consultation or a $150.00 deposit for consultation and same-day treatment.
STANDARD RESERVATION POLICY
A credit card number is required to secure all appointments.
PHOTO POLICY
We do require patients consent to use of clinical before and after photos before receiving a treatment. Consent reads as follows: I hereby irrevocably consent that my photo(s) or portrait(s) of me, or of any part of me, or reproductions therefore, may be used by Contempo Aesthetics and their employees for internal purposes only including producing before/after collages for my review only. Photos will not be shared via digital channels including social media and/or website. I hereby certify and represent that I am 18 years of age or older.
INJECTABLES FOLLOW-UP APPOINTMENTS
As of November 22, 2021, regardless of the number of "units" recommended, we have a minimum Botox or Dysport spend of $250 per treatment / visit / appointment in order to meet rising product, inflation, and service costs. For dermal filler recommendations of 0.5cc/mL or less, a $100 up-charge will be applied to the base pricing or $500 minimum treatment cost. Contempo Aesthetics offers clients the opportunity to schedule a follow-up appointment at a time recommended by Dr. Rachel post-treatment (typically at least 7-10 days for Botox or Dysport and 10-14 days for dermal filler). The follow-up appointment is meant to review before and after photos as well as address results and maintenance. If additional Botox and/or dermal filler is recommended, a 25% discount off the retail rate is applied. This is extended through the first 30 days post initial treatment(s). PRICES ARE SUBJECT TO CHANGE WITHOUT PRIOR NOTICE.
PRODUCT RETURNS
Your satisfaction is our utmost concern. If you wish to return a product, only the unopened item with a dated receipt within 30 days of purchase will be accepted. Please bring in the product with your receipt within (30) days of the purchase as the refund will be credited back to you in the same method of payment used for purchase.
CONTEMPO AESTHETICS RESERVES THE RIGHT TO REFUSE SERVICE TO ANYONE.
