Contempo Aesthetics Model Opportunities

If you are interested in working with Contempo Aesthetics on collaborative opportunities, we encourage you to read through the requirements below and book a consultation with Dr. Rachel here. Please note that this form is for model inquiries only.

Stipulations: 

1. You consent to the use of full face before and after photos as well as video of the service. 

2. You understand that a deposit will be collected in order to book the service and will be applied to the balance.

3. As a model, you will be entitled to receive said treatment with a 25% discount off retail rate. 

4. A portion of the paid service will be donated to a charity whose mission resonates with Dr. Rachel: Every Mother Counts

5. As a model, it is at the provider's discretion which service(s) and product(s) are used, quantity, and location(s). 

6. You must sign a model release consenting to use of photography in digital and print media.
